Participants will be staying at a four-star hotel in a mountain community setting and are responsible for bringing adequate and
appropriate attire for this health retreat. Lockdown will provide a check-list to facilitate the packing process. For this health
retreat, we want to create a safe and effective learning environment while minimizing any outside distractions; therefore, we would
ask that participants leave their cell phones at home or at least turned off and left in their hotel rooms.
Enrolled participants are responsible for traveling to and from the Zermatt Resort & Spa located in Midway, Utah. An address and
map will be provided. Lockdown Health & Fitness will not be held liable or responsible for any damages, costs, injuries, and/or
death that occur while traveling to or from the retreat location.
Lockdown Health & Fitness will provide participants with all meals and snacks during the course of stay at the health retreat. In
addition, all standard hotel accommodations (i.e. hotel room, fitness facility etc) are included in the advertised price of the health
retreat. Any additional and unexpected hotel charges such as pay per view or internet are not included in the retreat package price
and will be billed to participants for payment.
A deposit of [$200] is required at the time of booking to hold and secure your reservation. The remaining amount of the health
retreat is due 21 calendar days prior to your scheduled health retreat making the total payment in full. This serves as the
pre-registration process. Upon payment in full, Lockdown Health & Fitness will finalize the registration process via email,
telephone, and/or postal mail to confirm remaining retreat accommodations with the enrolled participant.

Cancellations and Refund Policy:
Cancellations must take place 21 calendar days prior to the health retreat to be eligible for a partially refundable deposit of
[$150]. The remaining [$50] of the deposit is non-refundable. However, if the cancellation results in a rescheduled retreat, the
non-refundable deposit may be applied to the final cost of your new health retreat reservation. There will be no refunds issued
for cancellations following the 21 calendar day designation.
